What is Frailsafe?


The Frailsafe project is supported by a Health Foundation ‘Closing the Gap in Patient Safety’ award. There were over 100 applications for the nine grants and the award represents a significant achievement for the Frailsafe team and recognition that quality of care for older people is a priority at the Health Foundation.

Frailsafe aims to improve measured quality of care for frail older patients admitted to NHS hospitals with medical emergencies by July 2016.

We will achieve this by learning how to effectively implement and spread a safety checklist, Frailsafe, using a collaborative of 12 hospitals learning together. We predict this will reduce harm by a combination of; improving communication, improving team working, reducing avoidable harm and improving reliability with evidence based care interventions.

The checklist is designed to be a ‘check and challenge’ list triggering an interaction between the senior doctor and another member of staff (eg nurse or junior doctor) on acute medical assessment units (AMUs).

Sites involved in the collaborative will be involved from August 2014 to August 2015 after which there will be national implementation of Frailsafe across acute hospitals in the UK.
